3 out of 5 liver cancers are preventable, new study discovers; cases could double in 25 years

Sixty percent of the 700,000 people who die each year from liver cancer could be prevented, reports a new study published in the journal Lancet.

That comprehensive research, according to a recent story by Nina Agrawal in The New York Times, finds that "prevention could be accomplished by addressing the disease's major causes: hepatitis B, hepatitis C, alcohol-associated liver disease, and liver disease linked to metabolic risk factors like obesity."

The Times article quotes Dr. Brian P. Lee, an associate professor of medicine at the Keck School of Medicine at the University of Southern California, who was not involved in the study, to the effect that "liver cancer is common, it causes immense suffering and death, and the saddest part for me as a physician is that most of the cases are preventable."

Liver cancer is the sixth most common cancer and the third leading cause of death each year from cancer. There are nearly 900,000 new annual cases globally, Agrawal's piece indicates.

The story also quotes Dr. Ahmed Kaseb, a professor of gastrointestinal medical oncology at the University of University of Texas MD Anderson Cancer Center, who also wasn't associated with the study, as suggesting that the threat of liver cancer from heavy alcohol use and metabolic dysfunction-associated steatotic liver disease, or MASLD, formerly known as nonalcoholic fatty liver disease, "has been under-recognized and underestimated." 

Dr., Hashem El-Serag, chair of the department of medicine at Baylor College of Medicine in Texas and one of the authors of the new study, maintains that a vast majority of liver cancers arise in people with cirrhosis, advanced and largely irreversible scarring of the liver, which is definitively linked to heavy  alcohol consumption.

Neither bras nor caffeine cause breast cancer

Article details risk factors for breast cancer while outlining preventive measures and dispelling myths


Are the causes of breast cancer known?

No, according to a feature story by Dr. Lizellen La Follette in the Marin Independent Journal that I've kept around quite a while because it's a good summary.

Both the article and its headline note, however, that although the roots of breast cancer aren't clear, the risk factors are.

La Follette details those factors: gender (being a woman); age ("two out of three with invasive cancer are diagnosed after age 55"); family, personal health, menstrual and reproductive histories; dense breast tissue; radiation to the chest; genetic changes; poor diet and alcohol consumption; obesity; lack of physical activity; smoking; and exposure to DES, a medication to prevent miscarriage that was heavily prescribed from the '40s through the '60s. 

Also, looking for dimpling or puckering of the breast, inversion of the nipple, redness or scaliness of the breast skin or nipple/areola area, or discharge of secretions from the nipple.

La Follette also dispels some myths about the disease — including the biggie, that it's contagious. It's not.

Other myths include that the disease can be caused "by wearing underwire bras, implants, deodorants, antiperspirants, mammograms, caffeine, plastic food serving items, microwaves or cellphones."

The article also suggests what women can do as prevention: having monthly breast cancer exams, checking for changes in breast tissue (such as in size), and feeling a palpable lump.

As for the risks, La Follette writes that a woman's "risk of breast cancer nearly doubles if she has a first-degree relative (mother, sister, daughter) who has been diagnosed" with it — "and her risk increases if a relative was diagnosed before the age of 50."

Furthermore, she notes, "studies show about 5 to 10 percent of breast cancers can be linked to one mutations inherited from one's mother or father."

Regarding smoking, the columnist contends that smoking is a danger "particularly when it's long-term, heavy and among those who started before their first pregnancy." Meanwhile, the risk "is about 1.5 times higher in overweight women, and two times higher in obese women than in lean women." 

In addition, La Follette maintains that "numerous studies confirm on average alcohol consumption increases the risk of breast cancer in women by about 7 to 10 percent for each drink consumed," and she writes that "women who have two to three alcoholic drinks a day have a 20 percent higher risk…compared with non-drinkers."

Finally, she postures that "early menstruation (before age 12), mate menopause (after 55), having first child at older age or never having given birth can be risk factors."

La Follette also takes a moment to mention males, citing statistics that while the average man's lifetime risk is about 1 in 1,000, African-American men "are hit harder by breast cancer than their white counterparts and after diagnosis are three times more likely to die" from it than white men.

Half of cancer deaths may be preventable

Even small lifestyle changes can sharply cut your cancer risk, NY Times columnist claims

Evidence, writes a medical columnist in The New York Times, "has increasingly accumulated that cancer may be preventable."

Especially if potential patients stop drinking alcohol and smoking tobacco, alter their diets, exercise and lose weight.

In a recent Aaron E. Carroll-bylined column under the rubric of The New Health Care received recently from The New York Times' online service, it's stated that "simple changes to people's behaviors have the potential to make sure many cancers never occur."

And they have "a side benefit of preventing health problems in many other areas, too," Dr. Carroll maintains.

The website headline indicates that "quite a bit is in your control" regarding the prevention of cancer, and the column, originally printed in 2016, cites a study published in Nature to the effect that "there is a lot we can do" to reduce our risk of cancer by changing our behavior.

That study contradicts research published in Science magazine in 2015 that many apparently took to mean that "cancer is much more because of 'bad luck' than because of other factors that people could control."

Carroll's piece contends that "many studies have shown that environmental risk factors and exposures contribute greatly to many cancers. Diet is related to colorectal cancer. Alcohol and tobacco are related to esophageal cancer. HPV is related to cervical cancer, and hepatitis C is related to liver cancer. And you'd have to be living under a rock not to know that smoking causes lung cancer and that too much sun can lead to skin cancer."

But, the professor of pediatrics at the Indiana University School of Medicine notes, "using sophisticated modeling techniques, the researchers argued that less than 30 percent of the lifetime risk of getting many common cancers was because of intrinsic risk factors, or the 'bad luck.' The rest were things you can change."

According to the study, "about 82 percent of women and 78 percent of men who got lung cancer might have prevented it through healthy behaviors. About 29 percent of women and 20 percent of meant might have prevented colon and rectal cancer. About 30 percent of both might have prevented pancreatic cancer."

Over all, it said, "about 25 percent of cancer in women and 33 percent in men was potentially preventable. Close to half of all cancer deaths might be prevented as well."

Data from 1.2 million females reviewed

Daily glass of wine or booze can dramatically raise women's risk of breast cancer, review shows


Having just one glass of wine or another alcoholic drink a day is likely to boost a woman's risk of breast cancer.

That's what a new massive review indicates, according to a story by Laurie McGinley in today's editions of The Washington Post.

The increased risk is considered significant.

Statistically, McGinley's piece says, the difference is "5 percent for pre-menopausal women and 9 percent for post-menopausal women."

Reducing risk is also possible, the story also reports, by vigorous exercise such as running and bicycling.

That information translates, according to the study, into "pre-menopausal women who were the most active [having] a 17 percent lower risk of developing malignancies compared to the least active women, while post-menopausal women had a 10 percent decreased risk."

Why the added or diminished risks?

McGinley writes that "alcohol increases estrogen, which is linked to increased breast-cancer risk, while physical activity reduces it."

The review — done by the American Institute for Cancer Research (AICR) and the World Cancer Research Fund — "evaluated research in 119 studies encompassing data on 12 million women from around the world."

AICR estimated that 1 in 3 cases of breast cancer could be prevented if women didn't drink alcohol, were physically active and maintained a healthy weight.

The Post article cites Anne McTiernan, a cancer-prevention researcher at Fred Hutchinson Cancer Research Center in Seattle and one of the report's lead authors, to the effect that the review "suggests there is no level of alcohol use that is completely safe in terms of breast cancer. If a woman is drinking, it would be better if she kept it to a lower amount."

The review showed, too, that women who are overweight or obese run a higher risk of post-menopausal disease in general. Losing just 10 percent of their weight apparently can reduce blood estrogen and inflammation.

But even a healthy lifestyle is no guarantee.

The Post paraphrased the researcher as saying it's more like wearing a safety belt — with many women doing everything they can to reduce their risks but still get diagnosed with breast cancer.

"That's unfortunate, but that's what happens," the newspaper quoted McTiernan as saying.
